Before coming to your appointment:

If your child is a new patient, please feel free to fill out the new patient paperwork online. This will help limit your wait time (which is something anyone with little kids or busy teenagers can appreciate!)

When you come for your appointment, please remember to bring the following:

• Copies of test results, X-rays, MRIs, CT cans, and any other information you feel may be helpful

• We like to look at the actual images ourselves when available. Please bring a copy of the images on a disc and, if available, a copy of the radiology report as well.

• Any current braces and/or orthotics

• Medication list (if any)

• Insurance card

• Your driver’s license or a valid form of identification

Should you need to cancel your appointment:

We get it, things happen! Please call our office during regular business hours or leave a message on our after-hours line to let us know. Try to give us at least one day’s notice when possible so we can offer your appointment time to a patient on our waiting list.
